The NHS Constitution
The NHS Constitution brings together, for the first time, the principles, values, rights and responsibilities that underpin the NHS.It is designed to renew and secure our commitment to the enduring principles of the NHS, making sure that the NHS continues to be relevant to the needs of patients, the public and staff in the 21st century.
Consent
Before a doctor or other health care professional examines or treats you, they need your consent. Sometimes you can simply tell them whether you agree with their suggestions. However, sometimes a written record of your decision is helpful - for example if your treatment involves sedation or an anaesthetic. You will then be asked to sign a consent form. Before signing, the doctor or nurse will explain what is going to happen. remember to ask questions if you are not sure or do not understand something. If you change your mind, you are entitled to withdraw consent, even after signing the form.
Confidentiality of Information
When you arrive you will be asked to complete a hospital patient form. This will be used as part of your medical record. We ask you for information about yourself so that you can receive proper care and treatment. We keep this information, together with details of your care, because it may be needed if we see you again. Details of your past and future treatment will be kept in your medical records. Some sections will be processed on computer, in accordance with the Data Protection Act 1988. Great care is taken to make sure that confidentiality is maintained.
Access to Your Medical Records
Under Data Protection, you have the right to copies of information held in your medical records. This may include medical photography, x-rays, appointment details or test results. If you would like a copy of the information held in your medical records, please apply in writing to:

We are always looking to improve our services and undertake audit of medical records from time to time. Please tell us if you do not want us to include your records in our audits.
The Constitution Contains:
- A short introduction, which outlines the purpose of the NHS and of the Constitution.
- The principles of the NHS, which are the enduring high-level 'rules' that govern the way that the NHS operates, and define how it seeks to achieve its purpose.
- NHS values - that inspire passion in the NHS and should guide it in the 21st century
- Rights to which patients, public and staff are entitled and are protected by law, pledges which the NHS is committed to achieve (but are not legally binding), and responsibilities which the public, patients and staff owe to one another to ensure that the NHS operates fairly and effectively.
